Palliative/Hospice Care
Comfort and pain relief are our priorities for our loved ones in their dying days. We work with you to understand you and your loved ones' wishes if you choose to spend your last days in the privacy of your home. We know that this is a difficult time for family and friends, and we seek to ensure that your final moments with your loved one are free from any burdens.
The goal of palliative hospice care is to prevent and relieve suffering and to support the best possible quality of life for patients and their families, regardless of the stage of the disease or the need for other therapies. This includes enhancing the quality of life for patients and family, and helping with decision making and providing opportunities for personal growth.
Palliative hospice care is delivered when the dying patient has less than 6 months to live (terminal cancer, end-stage renal failure), and desires to spend his or her dying days within his or her home, or in a facility (such as a nursing home) with the support of an interdisciplinary care team. We can collaborate with hospitals and their primary physician to provide medical care to the hospice patient when aggressive symptom management is needed, or when the patient is actively dying. We can also provide respite care as needed by the family.
Always There For You care providers have core assessment and family coping skills to ensure that clients and their respective families are cared for. Our palliative hospice care providers assist with patient and family communication to establish treatment goals, and may also see the patient and carry out orders for daily needs while maintaining a collaborative relationship with the family and the primary physician. They provide emotional support to family members, especially if it is their first time to be thrust into the responsibility with the care of the dying family member.
