One of our home health care specialists will meet with you and your family at your home. Our structured review identifies your unique needs and challenges. Our assessment specialist will review the following:

• Your medical and health care history and requirements
• Medical equipment provision: we can provide walkers, bed rails, canes, special beds
• Estimate for Home adaptation (electrical chair, ramp installation)

When required, we will meet with all your family members to ensure that all stakeholders' needs are satisfied.

Home safety assessment

Aside from your health conditions, we are also concerned for your safety in the home. We look for hidden dangers or obstacles in your home or environment, which we have included in our complimentary assessment

Customized care for your unique needs

At the end of this review, which typically takes up to an hour, we will create your own custom care plan.  Our plan will ensure that your caregiving team is made up of caregivers that will fit you, your health issues and your unique cultural needs.

We can give you resources for financing your home care

If and when needed, we can provide resources for funding as well as advice on suitable services provided through different federal or government agency programs.
